After entering your signs and symptoms regarding the issue you are looking to get checked out click the "Record Snapshot" button and you will get and visual aide to see where your symptoms are.

Once you have filled out all the information click the button at the bottom to save the information and you will get a pop up screen to save the file. this can be sent to your healthcare provider or printed to allow you to hand it to them in person.
